Acceptable Forms of Identification for the Application for Delivery of Mail Through Agent form (1583 form)

 

The US Post Office determines the forms of acceptable identifications and requires -2- forms of identification to rent a mailbox. 

 

Photo ID requirement:

 
  1. U.S. Driver's or Nondrivers ID Card
  2. Uniformed Service ID
  3. U.S. Access Card
  4. U.S. University ID Card
  5. Passport
  6. Matricula Consular (Mexico)
  7. NEXUS Card (Canada)
  8. Cerificate of Naturalization
  9. U.S. Permanent Resident Card
 
Address ID requirement:
 
  1. U.S. Driver's or Nondrivers ID Card
  2. Current Lease
  3. Mortgage or Deed of Trust
  4. Home or Vehicle Insurance Policy
  5. Vehicle Registration Card
  6. Voter Card

 

Unacceptable Forms of Identification
 
The Postal Service does not accept the following as forms of identification:
 
  1. Social Security cards
  2. Birth certificates
  3. Credit cards / debit cards
