Acceptable Forms of Identification for the Application for Delivery of Mail Through Agent form (1583 form)

The US Post Office determines the forms of acceptable identifications and requires -2- forms of identification to rent a mailbox.

2. Uniformed Service ID
3. U.S. Access Card
4. U.S. University ID Card
5. Passport
6. Matricula Consular (Mexico)
7. NEXUS Card (Canada)
8. Cerificate of Naturalization
9. U.S. Permanent Resident Card
