Acceptable Forms of Identification for the Application for Delivery of Mail Through Agent form (1583 form)
The US Post Office determines the forms of acceptable identifications and requires -2- forms of identification to rent a mailbox.
Photo ID requirement:
- U.S. Driver's or Nondrivers ID Card
- Uniformed Service ID
- U.S. Access Card
- U.S. University ID Card
- Passport
- Matricula Consular (Mexico)
- NEXUS Card (Canada)
- Cerificate of Naturalization
- U.S. Permanent Resident Card
Address ID requirement:
- U.S. Driver's or Nondrivers ID Card
- Current Lease
- Mortgage or Deed of Trust
- Home or Vehicle Insurance Policy
- Vehicle Registration Card
- Voter Card
Unacceptable Forms of Identification
The Postal Service does not accept the following as forms of identification:
- Social Security cards
- Birth certificates
- Credit cards / debit cards
